The Achilles tendon is an incredible structure. It connects the powerful calf muscles to the heel, allowing us to walk, run, jump and change direction with strength and control. It is the largest and strongest tendon in the body, capable of absorbing loads up to twelve times your body weight when running. With this much force passing through it, it is easy to see why Achilles pain is so common, particularly when activity levels change.
What is Achilles Tendinopathy?
Achilles Tendinopathy refers to pain and dysfunction at the back of the heel. It often occurs after a sudden increase in training load, such as beginning a running program, returning to sport, or increasing intensity too quickly.
There are two main types:
Mid-portion Tendinopathy
Pain 2 to 6 centimetres above the heel, in the middle of the tendon. This is the most common presentation.
Insertional Tendinopathy
Pain at the point where the tendon attaches into the heel bone.
People often describe morning stiffness, discomfort when standing after sitting, and pain that increases with running or jumping. Symptoms may ease as you warm up, then return once you cool down or the following day. The tendon may also appear thickened or swollen in the painful area.
What causes it?
Achilles Tendinopathy is commonly linked to:
- A rapid increase in training load or activity
- Sudden changes in footwear, especially minimalist shoes
- Reduced ankle mobility
- Weakness in the calf and lower-limb muscles
These factors place more stress on the tendon than it is ready to manage, leading to irritation and pain.

How do we treat it?

Your plan may include:
Load management
Temporarily reducing or modifying higher-demand activities such as running, hopping and jumping so the tendon can settle.
Graduated strengthening program
Strength training is the cornerstone of Achilles Tendinopathy recovery. Building calf strength and improving capacity ensures the tendon can tolerate day-to-day activity and return to sport safely.
